RECEIVE SHOES FOR CHILDREN IN YOUR CARE
We know that returning to school can bring all kinds of new stresses into your already stressful home. One of the ways we would like to support your family as your kids return to school is by providing a brand new pair of shoes to each school-age child in your home (3 yrs and older). That’s one thing you can check off your list!

WHAT KIDS QUALIFY FOR NEW SHOES?
• ALL children living in a foster home (including biological or adopted children), with a kinship caregiver, or a safety plan home.
• Must be school-age – entering pre-k (3 yrs old) thru 12th grade.
• Caregivers will be verified through agencies and DFPS caseworkers (you will include this info in your request).
HOW DO WE REQUEST SHOES?
• Fill out the form below with all information. Please do not leave any spaces blank.Â
• Caseworkers, CASA, etc. may request and pickup items for families. However, you will need to fill out the form using the family’s information (not your own). There will be a place to indicate who is filling out the form and who will be picking up. There is only one curbside pickup time for caseworkers and CASA workers.
WHEN/WHERE DO WE GET THE SHOES?
• There will be a shoe shopping event on Sunday, July 19th at Concordia Lutheran Church. Your family will choose a shopping time slot and each child will get to pick out their own shoes. The appointment time for caseworkers or CASA advocates to shop for shoes is 4:00pm-4:30pm.

• You can send anyone to pick up the shoes for you as long as they have a copy of your confirmation email.
